Pottawattamie County Community Foundation Supports

“Caring for Our Communities”

Pottawattamie County Community Foundation is excited to announce a grant award of $1,000 to Caring for Our Communities, a project of the Jennie Edmundson Memorial Hospital Foundation. This program assists older adults and their families with purchasing medications and reducing drug costs.

“The Caring for our Communities program provides necessary care and assistance to individuals in Pottawattamie County,” said Donna Dostal, president and CEO of the Pottawattamie County Community Foundation (PCCF). “Our focus is to connect individuals with outstanding programs, Caring for Our Communities is a prime example of the programs we strive to support at PCCF.”

Unfortunately, many seniors require multiple medications for chronic conditions and face heavy economic burdens associated with healthcare. Due to cost concerns, these patients tend to take less of their medication than has been prescribed. This is where Caring for Our Community can provide some assistance.

Caring for Our Communities provides many avenues of assistance, including:

· Assisting patients in accessing an appropriate level of healthcare

· Facilitating partnerships with behavioral health and community healthcare advocates

· Helping complex patients navigate the healthcare system

· Establishing self-management goals that will engage patients in their own healthcare

“The Caring for our Communities program is a collaboration between nine stakeholders, as well as thirty community partners,” said Tara Slevin, vice president of development at Jennie Edmundson Hospital Foundation. “We reach out for those who are high-risk, as well as people who are under-resourced. Our goal is to connect individuals we serve to community resources they need.”

The Pottawattamie County Community Foundation is a partnership of rural and urban citizens dedicated to improving the lives of all residents by supporting and stimulating donor-driven philanthropy. New endowment funds and future contributions will receive an additional 10% contribution from PCCF. Such additions are funded, in part, by the Iowa West Foundation.
